#1 - I forgot my username, what should I do?
Click on the link under the User Id sign-in box.
#2 - I forgot my password, what should I do?
Click on the "Forgot Your Password" link under the Password box
#3 - What is Healthkey?
Healthkey is a service that allows you to use the same user ID and password on multiple doctors' websites, making it easier to access your healthcare information.
#4 - Does your patient portal have a mobile app?
Yes. There is an app available for Android and iPhone devices.

For your convenience the Patient Portal has been redesigned to work in mobile devices, tablets, laptops, and desktop computers without installing an additionall app or any special software.
#5 - The portal is not responding, what should I do?
Our Patient Portal is managed by Intuit/Medfusion. If you experience problems connecting to Medfusion, please refresh and revisit the site in 30 minutes. If problems persist, please call our office and leave a message for our IT support department and they will be in touch with you within 48 hours.
#6 - Where can I find my visit summary?
Click on Patient Forms on the menu on the left or if you are on "My Patient Page" you can select Health Forms. Scroll to the bottom of the forms page and the visit summary will be under the visits tab.
#7 - How can I get a copy of my recent labs?
Select the "Ask a Question" option from the menu on the left, then select "Request Blood Work Results" and fill out the form.
#8 - How can I get a copy of a recent test I had done in your office?
Select the "Ask a Question" option from the menu on the left, then select "Request for Test Results" and fill out the form.
#9 - If I am not feeling well or have a general medical question can I use the Portal to reach the office?
If you are not feeling well, please call the office to speak with a nurse. If you have a general medical question of a non-urgent matter, please select the "Ask a Staff Member" option and state your question. You can expect a response within 48 hours. (We are closed on Holidays and Weekends.)
#10 - How quickly can I expect a response to my request?
You should expect a response within 48 hours. Remember, we are not open on weekends or major holidays.
#11 - What should I do if I am experiencing a medical emergency?
Please call 911 if you are experiencing a medical emergency. DO NOT USE THE PORTAL FOR ANY URGENT ISSUES.
